Attitudes and behaviour are a hugely important component in assessing the performance of a trainee. These are best assessed in the work setting by those who regularly work with the trainee. The 360 form enables us to spot trainees who may have a problem, and to congratulate those who perform well. You are one of at least 10 assessors whose reports will be considered.Filling the form
- Only fill in the sections on the form where you have had a chance to observe the trainee enough to make a judgement.
- Let your scoring and comments reflect the typical behaviour over time.
- Bear in mind that this is not so much about whether you like the trainee, but rather about how they perform.
No concern
For the majority of trainees, the purpose of the free text box is to congratulate and praise good behaviour.
Some concern
Tick this box if you have a concern, if a few incidents or behaviours have worried you. This is about helping them to address blind spots. Describe the behaviours which have caused you concern in the free text box (and overleaf if needed).
Major concern
