REGISTERING FOR APPRAISAL
To register for appraisal it is necessary for you to send in a Registration and Declaration Form (see Appraisee Pack) to the Appraisal Office in NIMDTA. To download a registration form please follow these instructions:
- Click the required Registration and Declaration Form link from the Appraisal Pack below. This will open the form.
- Save the form onto your computer by File and Save As, giving it a name you will recognise.
- Close the form and close the website.
- Complete the form you have saved on your computer by typing on the grey boxes (it is advisable to save as you go along).
- Save and close the form.
- When completed, please send the form, as a word attachment only, by e-mail to gpappraisal@nimdta.gov.uk.
- An e-mail will be sent confirming receipt of this form within a couple of days. If you do not receive this, it is important that you contact the Appraisal Office on tel: 9040 0020 / 32 / 21.

CONTACTING APPRAISERS
On receipt of your Registration and Declaration Form, NIMDTA will contact one of your requested appraisers on your behalf. They in turn will contact you to arrange a suitable date, time and venue. Please do not arrange an appraisal date with an appraiser before sending in your GP Appraisal Registration and Declaration Form.
APPRAISAL DOCUMENTATION
Other documents in the Appraisee Pack are used to either prepare for, or act as a basis for discussion at appraisal. Appraisers will be able to guide you in the completion of your documentation and advise on the most suitable supplementary information to be attached. Please send this information directly to your appraiser four clear weeks before the date of your appraisal.
AFTER THE APPRAISAL
The following forms are to be returned within two weeks of the appraisal meeting. The Appraiser will organise that all parts of the appraisal documentation are signed off.
- Form 6
This form will be forwarded by the Appraiser and confirms that the appraisal meeting has taken place and that both the Appraisee and Appraiser are satisfied with the process that has taken place. - Form 6A
This form will be forwarded by the Appraiser and will be used for the collation and subsequent dissemination of the educational and service needs identified. - Appraisee Feedback Form
This form will be forwarded by the Appraisee. The Appraiser will also complete and forward a feedback form.
