1 - Complete and submit a Registration and Declaration Form.
To download a registration form:- Click on the link above and save the form onto your computer by File and Save As. Close the website page.
- Complete your saved form by typing on the grey boxes.
- Save and Send the form, as a word attachment only, by e-mail to gpappraisal@nimdta.gov.uk
- Within a few days you will receive a confirmation email. Please contact us on 02890 400021/32 if you do not receive this.
On receipt of your Registration and Declaration Form, NIMDTA will contact an appraiser on your behalf. Your appraiser will contact you to arrange a suitable date, time and venue. Please do not arrange an appraisal date with an appraiser before sending in your GP Appraisal Registration and Declaration Form.
3 - Appraisal documentation (Forms 1 to 6A) via DHSSPS website (Complete and send to your appraiser 4 weeks before appraisal)

4 - Probity form
6 - Appraisees informing NIMDTA of extenuating circumstances - Please use this letter should you wish to inform NIMDTA of the possibility of extenuating circumstances. Please be aware you will also need to inform the HSCB who hold responsibility for extenuating circumstances. This letter can be emailed to gpappraisal@nimdta.gov.uk or posted to NIMDTA.
7 - Form CP (2A) - This form should be completed by GPs employed in extended roles. It should be completed by you and your employing authority. A different form should be completed for each role you are employed in. If you are unable to complete Form CP (2A) you should complete a self declaration - Form CP (2B) .
CP (2A) or CP (2B) should be forwarded to your appraiser with your pre-appraisal documents OR given to your appraiser at your appraisal. We would ask that you retain these forms following your appraisal; the appraisal team at NIMDTA do not require a copy.

11- PDP Guidance - Please bring with you to your appraisal an outline PDP for the next year.
The documents in the appraisee pack are used to either prepare for, or act as a basis for discussion at appraisal. Your appraiser can advise you on the completion of these and the most suitable supplementary information to be attached.
AFTER YOUR APPRAISAL
The following forms are to be returned within two weeks after your appraisal.

Form 6
Your Form 6 will be forwarded to NIMDTA by your appraiser. It must be signed and dated by both yourself and your appraiser to confirm your appraisal has taken place. -
Form 6A and Service Needs Survey
A Form 6A and Service Needs Survey should be completed jointly by you and your appraiser after the appraisal discussion. If you are unable to complete this online after the appraisal discussion you should complete it at a later date. All Form 6As are collated and determine GP CPD. The service needs survey is collated and the information is forwarded to the HSCB for appropriate action.
