When should I complete my GP ST3 appraisal?

You must complete your GP ST3 appraisal before the end of your ST3 Year. You should complete your appraisal after successful progression through an Annual Review of Competence Progression (ARCP) panel and before receiving your Certification of Completion of Training (CCT).

When should I complete my next appraisal with NIMDTA?

You are required to complete your next appraisal within 12 calendar months of the completion of your GPST3 appraisal. Therefore for the majority of trainees who have completed their appraisal in August 2011, this will mean completing your appraisal before the 31st August 2012. You will be able to book your appraisal from 9th January 2012.

Who can complete my GP ST3 appraisal?

Your trainer should complete your appraisal with you, or alternatively another partner within your training practice, who is an appraiser.

What documentation must I complete prior to my appraisal?

Before your appraisal you should have completed Forms 1-3. You should forward these to your trainer a minimum of 2 weeks prior to the appraisal discussion. ALL FORMS MUST BE TYPED - HANDWRITTEN FORMS WILL NOT BE ACCEPTED

  • Form 1 - Detailing your basic details
  • Form 2 - Current medical activities
  • Form 3 - Material for appraisal

What documents need to be returned to NIMDTA following appraisal discussion?

The following completed documents should be returned to NIMDTA. Please email them to gpappraisal@nimdta.gov.uk. You will then receive an appraisal certificate by email.

  • Form 3 (written by trainee)
  • Form 4 (written by trainer and agreed by trainee)
  • Form 4 Sign off (signed and dated by both trainee and trainer)
  • PDP (written and agreed jointly by both trainee and trainer)
  • Form 6 (signed and dated by both trainee and trainer)

You and your trainer should complete an electronic Form 6A together - 2011/12 FORM 6A

Please do not submit the old hard copy Form 6A on the DHSSPSNI website.

How does appraisal differ from your final educational supervisors report?

GP appraisal is different from the educational supervisors report. The educational supervisors report focuses on the educational opportunities during your specialty training. GP appraisal, on the other hand, focuses on the development of your skills as a GP. There will be an educational element and parts of your eportfolio will be relevant to it but it is much more focused on the service delivery.
