1. Please complete a Registration and Declaration Form.

    Email your Registration and Declaration Form to gpappraisal@nimdta.gov.uk

2. Post £550 payment (Cheques should be made payable to NIMDTA) to GP Appraisal Department, NIMDTA, Beechill House, 42 Beechill Road, Belfast, BT8 7RL. Your application will not be processed until payment has been received

3. An Appraiser will contact you to arrange a suitable time and date for your appraisal

4. Print off your appraisal documentation, Health Form and Probity Form. Forward your appraisal documentation to your Appraiser at least 4 weeks prior to your appraisal date

5. Your Appraiser will forward a copy of your Form 6 to NIMDTA to enable us to send you an Appraisal certificate
