Absence Notification Procedure

In order for the Deanery to accurately track the amount of days a trainee has taken as sick leave, the below form has been developed.

Foundation Absence Notification Form

This is to help us in providing support, ensuring trainees have met their competencies and applying National guidelines regarding reviewing days of training lost to sick leave.

We do not need to know the reason for the sick leave but we do need to know the amount of time missed that a trainee was due to be in work (not to include scheduled days off).

This form should be sent to as soon as possible upon your return. Once we receive this, we will update your Turas system to show the number of days off under the Out of Programme section. You can also upload the completed form to the Turas System.
